Berks Counseling Center is Hiring a Health Coordinator Near Reading, PA

Description

This position is an active member of the integrated care team. This position supports the client in completing an integrated clinical assessment and is responsible for physical health screenings, including collection of health vitals, assisting in acquiring a primary care physician, communicating with pharmacies and community providers to ensure collaboration and coordination of care, referrals to specialists, and other duties as assigned.

Essential Job Functions

  • Monitors patients’ health needs by completing and updating health screenings. This may also include taking vital signs; height and weight; reviewing and/or recording medical history.
  • Facilitates referrals to internal and external health providers.
  • Works with patients to ensure engagement in services with a primary care physician.
  • Collects urine drug screens, log the specimens, and prepare paperwork for clinical staff to review for intake.
  • Completes medical records by recording patient examination, treatment, and test results. 
  • Assists with obtaining medical records from previous providers to ensure continuity of care.
  • Participates in daily huddles sharing relevant data and facilitating patient access to care.
  • Monitors assigned team’s population health and coordinates with health coach to develop team interventions. 
  • Conducts self in accordance with BCC employee manual. 
  • Maintains strictest confidentiality and adheres to all HIPAA and OSHA guidelines/regulations.
  • Keeps equipment operating by following operating instructions, troubleshooting breakdowns, maintaining supplies, performing preventive maintenance, and calling for repairs.
  • Updates job knowledge by participating in educational opportunities; reading professional publications.
  • Develops an understanding of trauma and how it impacts others. Incorporates trauma informed practices into job duties and responsibilities, including interactions and communications with others.
  • Is an active member of BCC’s culture of caring through positive and engaging social interactions. 
  • Demonstrates a commitment to diversity, equity, and inclusion. 
  • Demonstrates skill in understanding of cultural differences. 
  • Performs related duties and responsibilities as required.

Requirements

  • Associate’s degree in public health, exercise physiology, nursing, dietary or a related field required
  • Bachelor’s degree preferred. 
  • Minimum experience of two (2) years in health promotion, wellness, or health education is required. 
  • Certifications in health coaching from an accredited professional or institute is also preferred. 
  • Must have a valid PA Driver’s License and the ability to drive, travel, and provide community-oriented services. 
  • Knowledge of or experience with trauma-informed approaches preferred. 
  • Minimum of one (1) year training or experience relevant to working in a medical office with mental health or substance use treatment preferred. 
  • Basic Life Support certified.
  • Bilingual: English/Spanish preferred. Knowledge of or experience with trauma-informed approaches preferred.

  • Knowledge of routine medications often prescribed in integrated care settings. 
  • Knowledge of health risk factors associated with chronic health conditions. 
  • Knowledge and understanding of trauma and how it impacts others.
  • Skill in active listening and collaborative communication. 
  • Verbal communication skills in both Spanish and English preferred. 
  • Must be adept at supporting the health care and wellness needs of low-income high-risk populations. 
  • Must demonstrate a commitment to the provision of compassionate, patient centered care supportive of the patient’s own goals and motivated toward positive health outcomes. 
  • Must possess strong computer skills, be highly organized. 
  • Ability to provide culturally competent care to diverse populations.
  • Ability to effectively manage electronic health records. 
  • Ability to work collaboratively as part of a multidisciplinary team to meet the physical and behavioral health needs of patients served.
  • Ability to be flexible and adaptable to change.
  • Ability to engage in honest, transparent, and respectful interactions with others.
  • Ability to act with integrity and work from a place of empathy and compassion.

Physical Requirements  

SEEING: Must be able to see well enough to read reports and use computer.

HEARING: Must be able to hear well enough to communicate to clients, vendors, and coworkers.

MOBILITY: Must have mobility to move about the agency. Must have mobility to move about the community. LIFTING/PULLING/PUSHING: Must be able to lift/pull/push up to 15 pounds.

OTHER: Must be able to write, type and use technology.
