Greater Lakes is a CARF accredited community mental health center that offer a variety of Mental health services for residents of Pierce county. With an affiliation with MultiCare our services include mental health treatment for Adults and Children as well as Substance Use Disorder treatment for adults, court and criminal justice programs, homeless support programs, integrated medical care and a full service pharmacy.
Job Description
Greater Lakes PACT program focuses on individual’s recovery, safety in the community, the promotion of wellness and independence of the individual in their residential structured setting, and the re-establishment to a higher level of social life functioning. These services are designed to support and rehabilitate individuals who are experiencing symptoms of severe mental illness that impact their ability to live in the community. Seventy-five percent or more of services are provided outside of the program office in locations that are comfortable and convenient for clients.
This position provides direct clinical services to clients and provides Mental Health Professional (MHP) supervision to non-MHP members of the PACT Team. When appropriate, works to reduce reliance on ER’s and use of both medical and psychiatric inpatient hospitals. Assists clients with developing and using natural supports, with obtaining and maintaining stable living situations, obtaining necessary medical care and with the development of healthy and positive coping skills. As necessary, provides crisis intervention and support to PACT clients in the community. Responsible for rotating on-call shifts as assigned.
Responsibilities
1. Provides MHP level supervision to non-MHP staff.
Assures that client diagnoses and treatment plans comply with requirements of all appropriate funders.
Meets all of the documentation requirements as per agency policies, funder requirements and the WACs.
Supports, coaches and trains non-MHP staff on documentation, diagnoses and clinical interventions.
2. Provides effective clinical case management services to improve PACT clients’ ability to live successfully in the community.
Performs on-going clinically appropriate assessments of consumers.
Develops, with consumer voice, the goals to be addressed in individual treatment plans, tailored care plans, and crisis plans, in accordance with the appropriate model of care.
Provides behavioral counseling, crisis and stabilization services to consumers.
Maintains professional clinical boundaries with consumers.
Provides case management, and referral as needed.
Maintains clinical competency and resource knowledge required to meet changing consumer, community, and industry trends.
Attends treatment planning meetings and daily team meetings.
Assures confidentiality is always maintained.
3. Completes clinical documentation in a timely, accurate manner.
Develops individualized Recovery Plans with each client, including measurable goals and objectives for recovery and wellness, with a focus on reducing recidivism.
Review and update Recovery Plans with each client as frequently as needed, but at a minimum every 180 days.
If a Crisis Plan is needed, formulates it with the client, addressing all required elements.
Completes Discharge Summaries in a timely manner, as clinically appropriate.
4. Completes progress notes on time for each service providing a clear, concise description of the client’s progress towards treatment goals, being sure to document any risk concerns.

Progress notes routinely meet medical necessity requirements.
5. Practices effective caseload management in ways that meets productivity expectations.

6. Completes administrative documentation in a timely and accurate manner.
Completes and submits Time Sheets, Time Off Requests, Mileage Reimbursement forms, etc. accurately and on time.
Provides all required annual documentation to Human Resources and completes all required Relias Learning training in a timely manner.
Job Scope
Mental Health Professionals work semi-independently managing a caseload of consumers in a highly competent and professional manner. They also may coach and support non-MHP staff and may sign off on non-MHP documentation. Work involves a significant variety of non-routine tasks of moderate complexity which are guided by formal policies and general procedures. While this position has limited direct budgetary accountability, errors in work can have significant impact on customer service, clinical outcomes, and liability implications.
Supervisory Responsibility
Some incumbents may occasionally supervise interns.
Communication / Customer Contacts
This position involves a high frequency of communication, in various forms, verbal and written, with clients, other staff, and community partners. Must communicate on a regular and timely basis with supervisors, other staff, consumers, their families and community supports, and with professionals outside the agency. Must also abide by all confidentiality laws and pertinent codes of conduct. Use the computer system to send and receive email, to write reports, to schedule and complete clinical transactions. Utilizes voice mail and email on a regular basis, and responds to those contacts in a timely manner.
Knowledge and Skills
Familiarity with the DSM-V and ability to apply diagnostic criteria
Knowledge of the major components of a mental status examination and competency in performing on-going assessments
Ability to develop and implement treatment strategies tailored to individual client needs
Knowledge of community resources
Ability to assess training needs of team members and provide the training and coaching when needed
Well-developed skills in managing time, priorities, and paperwork; excellent written and oral communication skills
Must be able to work effectively in a team environment and the ability to collaborate effectively with external partners
Understanding of the importance of clear, professional boundaries, and maintenance of such
Cultural competency
Education and/or Experience
Master’s degree in a mental health related field and two years’ relevant experience. Must be a recognized Mental Health Professional as defined in WAC 246-341-0200/RCW 71.05020. Must be eligible for a Washington State Department of Health Agency Affiliated Counselor Registration or other licensed mental health credential.
Job Conditions
Must be able to drive a car; possess a good driving record; provide proof of auto insurance; be able to work both in and out of the office and go into consumers’ residences, which may require climbing stairs and walking on uneven terrain. Must be able to speak and hear at normal conversational levels.
