The Palliative Care Nurse Practitioner Supervisor is responsible for overseeing the delivery of palliative care services and providing clinical leadership to a team of Nurse Practitioners (NPs) and other interdisciplinary team members. The Supervisor will collaborate closely with physicians, nurses, social workers, chaplains, and other healthcare professionals to ensure the provision of high-quality, patient-centered care that aligns with organizational goals and best practices.

Key Responsibilities:

• Provide clinical supervision, mentorship, and support to Nurse Practitioners and other team members in the delivery of palliative care services.
• Lead interdisciplinary team meetings to review patient cases, develop care plans, and coordinate care across settings and disciplines.
• Conduct comprehensive patient assessments, including physical, psychosocial, and spiritual dimensions, to develop individualized care plans and treatment goals.
• Collaborate with attending physicians and consulting specialists to manage complex symptoms, address treatment goals, and optimize the overall quality of life for patients.
• Participate in education and training initiatives for staff, including orientation programs, in-service sessions, and continuing education opportunities.
• Ensure compliance with regulatory standards, accreditation requirements, and organizational policies related to palliative care services.
• Monitor key performance indicators (KPIs) and quality metrics to evaluate program effectiveness, identify areas for improvement, and implement performance improvement initiatives.
• Serve as a liaison between the palliative care team, patients, families, and external stakeholders to facilitate communication, resolve conflicts, and address concerns.

Requirements:

• Master's degree in Nursing (MSN) from an accredited Nurse Practitioner program.
• Current licensure as a Nurse Practitioner (NP) in the state of NY.
• Board certification as an Adult-Gerontology or Family Nurse Practitioner with a specialization in Palliative Care preferred.
• Minimum of 1 year of clinical experience as a Nurse Practitioner, with demonstrated expertise in palliative care and end-of-life management.
• Previous leadership or supervisory experience in a healthcare setting, preferably in palliative care or hospice.
• Strong clinical assessment and critical thinking skills, with the ability to manage complex medical and psychosocial issues.
• Excellent communication and interpersonal skills, with the ability to collaborate effectively with interdisciplinary team members, patients, and families.
• Commitment to providing compassionate, patient-centered care and advocating for the needs and preferences of patients and families facing serious illness.

Please note- The annual base salary range is $100k to $130k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
