Essential Duties

• Assisting clients with activities of daily living (ADL) such as bathing, grooming, dressing, and mobility.

• Providing companionship, emotional support, and occasional social outings for clients.

• Monitoring and documenting vital signs, such as blood pressure, heart rate, and temperature.

• Assisting with medication management, ensuring that clients take medications as prescribed and refilling medication containers as needed.

• Collaborating with the interdisciplinary team of healthcare professionals, including registered nurses, physical therapists, and occupational therapists, to provide comprehensive care for clients.

• Performing light housekeeping duties, such as cleaning and laundry, to maintain a clean and safe living environment for clients.

• Maintaining accurate, up-to-date records of client care, condition, and progress for both the client and care team.

• Providing ongoing communication and updates to the nursing supervisor and other healthcare team members as needed.

Minimum Requirements

• Strong interpersonal and communication skills with the ability to work effectively with clients, family members, and the healthcare team.

• High school diploma or GED required

• Excellent observation and problem-solving skills.

• Proof of COVID vaccination required

• Demonstrated commitment to providing compassionate, patient-centered care.

• Basic computer and documentation skills.

• Must be 18 years old or older

• Must pass a thorough background check and drug screening.

• Current CPR/First Aid certification.

• Possess a valid driver's license and reliable transportation.
