Essential Duties
Assisting clients with activities of daily living (ADL) such as bathing, grooming, dressing, and mobility.
Providing companionship, emotional support, and occasional social outings for clients.
Monitoring and documenting vital signs, such as blood pressure, heart rate, and temperature.
Assisting with medication management, ensuring that clients take medications as prescribed and refilling medication containers as needed.
Collaborating with the interdisciplinary team of healthcare professionals, including registered nurses, physical therapists, and occupational therapists, to provide comprehensive care for clients.
Performing light housekeeping duties, such as cleaning and laundry, to maintain a clean and safe living environment for clients.
Maintaining accurate, up-to-date records of client care, condition, and progress for both the client and care team.
Providing ongoing communication and updates to the nursing supervisor and other healthcare team members as needed.

Minimum Requirements
Strong interpersonal and communication skills with the ability to work effectively with clients, family members, and the healthcare team.
High school diploma or GED required
Excellent observation and problem-solving skills.
Proof of COVID vaccination required
Demonstrated commitment to providing compassionate, patient-centered care.
Basic computer and documentation skills.
Must be 18 years old or older
Must pass a thorough background check and drug screening.
Current CPR/First Aid certification.
Possess a valid driver's license and reliable transportation.
