As we continue to expand our services, we are seeking a dedicated Patient Care Coordinator to join our team and play a pivotal role in coordinating care for our clients.

Job Description:

The Patient Care Coordinator for Adults with Intellectual Disabilities will be responsible for overseeing and coordinating the care and support services for our adult clients with intellectual disabilities. This role requires a compassionate and organized individual who is committed to advocating for the well-being and empowerment of individuals with special needs.

KEY RESPONSIBILITIES:
Care Coordination:

  • Collaborate with healthcare professionals, social workers, caregivers, and other stakeholders to develop and implement individualized care plans for clients.
  • Assess the needs of clients and ensure that appropriate services, resources, and support are provided to meet those needs.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as necessary to ensure optimal outcomes for clients.

Advocacy and Support:

  • Serve as an advocate for clients, ensuring that their voices are heard and their rights are respected in all aspects of their care.
  • Provide emotional support and guidance to clients and their families, helping them navigate the healthcare system and access necessary services.
  • Assist clients in developing skills for daily living, independence, and self-advocacy.

Education and Training:

  • Provide education and training to clients, caregivers, and staff on topics related to intellectual disabilities, healthcare management, and advocacy.
  • Stay informed about current best practices, research, and developments in the field of intellectual disabilities and healthcare coordination.

Support Services and Training:

  • Offer support services and training to individuals with developmental disabilities who wish to reside in a home of their own.
  • Provide education and support in areas such as meal planning, grocery shopping, cooking, household maintenance, and scheduling and attending medical and dental appointments.
  • Assist clients in finding affordable and safe housing, as well as compatible roommates.
  • Educate clients on utilizing public transportation and provide support in navigating transportation systems.

Qualifications:

  • Previous experience working with adults with intellectual disabilities or other developmental disabilities.
  • Strong interpersonal and communication skills, with the ability to build rapport and establish trust with clients and their families.
  • Knowledge of community resources, support services, and healthcare systems for individuals with intellectual disabilities.
  • Ability to work collaboratively as part of a multidisciplinary team.
  • Empathy, patience, and a genuine commitment to promoting the well-being and dignity of individuals with special needs.
