Alternative Family Services, a Foster Family, Adoption and Mental Health Agency serving children and families throughout the Bay Area, has a part-time position for an Intensive Care Coordinator (ICC). Our Alameda County based Katie A program is designed to provide children in the child welfare system with community-based case coordination to ensure that the youth and family needs are met. This program is based on a culturally competent, family-centered, strength-based approach to treatment. Services rendered will be in the East Bay. About Alternative Family Services
Responsibilities
- Facilitate Child and Family Team meetings with child, family, informal supports, County Workers and providers present to work as a team to meet child and family goals
- Engaging community support for families
- Provide clinical case management, advocacy, linkage, support for clients and families
- Write progress notes daily to document delivery of service
- Maintain client charts to ensure compliance with Medi-Cal regulations
- Visit client sites and transport clients using personal vehicle that is maintained in safe working order into a variety of community settings
- Provide emergency and crisis response. Evenings and weekends may be required
- AA, BA, or MS required with the following amount of relevant experience
- AA 6 years’ experience
- BA 4 years’ experience
- MS 2 years’ experience
- Obtain and maintain fingerprint and government and Agency required background clearances
- CPR/First Aid certification
- Able to drive and transport children into a variety of community settings using a personal vehicle that is maintained in safe working order
- Current and valid California driver’s license with an acceptable driving record, current and valid auto insurance showing as an insured driver on the policy and vehicle registration
- If applying for Spanish-speaking pay differential, must be able to meet linguistic needs of families
- Skills in: cross-cultural communication, conflict mediation, reflective listening in a group setting, youth and family engagement
- Experience with meeting facilitation
- Experience with in-home or community-based services for children and families
- Experience in social services and non-profit organizations preferred
- Experience with EPSDT Medi-Cal funded programs is a plus
- Experience with WRAP services is a plus
- Knowledge of community-based programs and resources in the general Bay Area is a plus
- Experience with use of technology including managing calendars, accessing electronic health records, and using telehealth platforms to deliver high quality services to families (Zoom, MS Teams)
