Job Title: Care Manager
Job Summary:
As a Care Manager your function is to act as a leader in providing appropriate care management services to approximately 50 active clients according to the guidelines set out in the ACCESS Program Manual (and as required by the NYSDOH – HH, AIDS Institute).

The qualifications listed below are representative of the knowledge, skill and/or ability required. *Direct Care Management experience includes verifiable full/part-time care management or case work with the following populations: those with HIV/AIDS, or a history of mental illness, homelessness, or chemical dependence. Experience with families or groups preferred; experience with HRA, HASA, hospitals or clinics a plus. Qualifying experience does not include activities or job positions such as pastoral counseling, counseling, therapeutic activities, internships or any unpaid/volunteer positions.
