Job Title: Outreach Health Worker/BOOM!Health

Job Summary:

As a key member of the Care Management team, OHW’s provide a wide range of direct and indirect support services to participants and their families’ collaterals with the goal of assisting participants achieve positive outcomes and enroll potential participants.

Primary Duties & Responsibilities include, but are not limited to:

• Be familiar with the Argus/ACCESS Manual. Be familiar with Health Home guidelines.
• Uphold Argus Community’s Policies and Procedures according to the Personnel Policies and Procedures Manual.
• Be on time and attend all required meetings include weekly OHW meetings, bi-weekly staff meetings and trainings as scheduled.
• Conduct Outreach Engagement activities to participants from the DOH lists as they are referred.
• Encourage client empowerment through peer education and support: documenting such support.
• Provide diligent outreach attempts to locate hard-to-reach clients and/or clients loss to service.
• Conduct home and field visits i.e. Hospital and other agency visits to thoroughly complete intake packet and consents.
• Assist participants in getting to appointments with outside providers providing Escorts and or arranging transportation services.
• Assist Care Management Team in collecting initial assessments/reassessments regarding client’s information.
• Ensure client is suitable for admission to the program. (Criteria is Met).
• Check client Medicaid card to ensure clients Medicaid is currently active.
• Document accurate and precise information given by client to ensure appropriate care when case is transferred to Care Manager.
• Schedule appointments and referrals for client if needed at the time of intake.
• Explain the Argus Community Inc. Access program and the services provided.
• Travel throughout the assigned coverage area to ensure clients are being outreached
• Other client team activities as assigned by the Health Home Operations Manager including, but not limited to, the maintenance and organization of client charts ensuring that all release of information up to date and the scheduling of client appointments.

Qualifications/Criteria:

• Minimum of one-year full-time work experience; preferably in community outreach, social services and/or sales.
• Knowledge of community resources and experience in traveling within the community.
• Knowledge of HIV/AIDS transmission and treatment.
• Ability to travel within the five boroughs of New York City via public transportation.
• Meets physical requirements needed to complete assigned tasks and duties of the position with reasonable accommodations being made for individuals with disabilities.
• Empathy towards and a desire to work with persons with HIV/AIDS.
• Bi-lingual in Spanish preferred.
• Ability to communicate in a warm, caring manner and to motivate others.
• Basic writing ability a must.
• Knowledge of computers word processing a plus.

• High School Diploma/GED.
