Job Description
Job title: Social Worker
Position reports to Director. Youth Skills Building Program
Job Summary:
The Social worker will be responsible for adherence to the evidence-based program, collaborating with the Director to ensure compliance and reporting outcomes, and program needs.
Job Responsibilities:
Recruit Teens from immediate and at-large communities.
Schedule and conduct a tour of the facility for potential clients.
Maintain a waiting list and ensure that follow-up is ongoing.
Initiate procedure for follow-up of potential clients.
Maintain enrollment data of all Adolescents for reporting.
Provide all new families with orientation.
Acquire data necessary for the completion of enrollment in accordance with governing standards.
File/Case Management:
Accurately complete paperwork for enrollment of each adolescent for statistics.
Review written documentation in each child's file monthly and maintain it in accordance with standards.
Participate in regularly scheduled ongoing training and supervision.
Develop and maintain community partnerships, including relationships with juvenile justice agencies, other service providers, and community resources.
Additional duties may be assigned.
Other Job Functions:
Knowledge /Skills/ Abilities:
Bachelor's degree in social work or human services preferred.
Experience working with adolescents for a minimum of 3 years.
