Willow Terrace Nursing and Rehabilitation Center -

Responsible for providing in-house social work services, interdisciplinary care planning and assessment, admission day procedures, and discharge planning.

ESSENTIAL REQUIREMENTS


Bachelor's Degree in Social Work a school accredited by the Council on Social Work Education preferred. At least one year experience in a health setting or with the aged is desirable.

Must have knowledge of social casework principles and methods.

Must possess the ability to make independent decisions when circumstances warrant such action.

Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.Must possess leadership ability and willingness to work harmoniously with and supervise professional and nonprofession personnel

Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning.

Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing casework practice.

Must possess the ability to exercise sound judgment. And an aptitude for problem solving.

Must possess knowledge of community resources.

Employment is contingent on successful completion of a pre-placement physical.

Employment is contingent on completion of a criminal background check and Medicaid Exclusion Screening, both indicating negative results.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Provide in-house social work services to residents and their families.

Demonstrates knowledge and ability to perform psychological evaluations, counseling, crisis intervention, and social histories/and cognitive/mood testing. Has knowledge of and ability to assist residents and/or families with community resources.

Communicates and advises all transferring facilities and/or family of admissions requirements as established by Medicare, Medicaid, and admission policies of the facility

Demonstrates knowledge and ability to assist residents and/or families to apply for financial assistance, as needed to pay for medical care by the facility

Help residents and families to understand, accept and aid in following recommendations planned to restore residents to optimum social, emotional and physical adjustment within their capacity.

Stimulate case review and discussion with physicians and nursing staff residents progress and appropriate plans on a periodic basis.

Demonstrates knowledge and ability by insuring that historical information, current situations, problematic areas and current behavioral manifestations are substantiated, verified or contradicted and are accurately and clearly recorded in the Department record and summarized in the Medical Record.

Reviews patients Medical Record and documentation of progress notes. Maintains appropriate communication with residents responsible parties in relation to rehab status.

Coordinate and participate in scheduled interdisciplinary care conferences; contribute to the ongoing plan and provision of care on the part of the interdisciplinary team.

Complete MDS, care plans, and assessments on each resident as indicated.

Effectively coordinates for Resident discharges with supporting documentation.

Prepare case summaries and keep social service progress notes on each resident, utilizing problem oriented recording techniques.

Participate in the planning, delivery, and evaluation of facilities programs, policies, and procedures as part of the caseworker role as resident advocate.

Comply with department and facility procedures, fire prevention and safety standards.. Participate in bi-monthly department meetings for the purpose of `learning, sharing, and problem solving.

Participate in individual supervision with department Director for the purpose of training professional growth.

Compile monthly statistics, indicating direct resident and family involvement, to be turned into the Director.

Attend appropriate in-services.

Knowledge of, commitment to, and identification with the agency's objectives.

Work within limits of agency's policies and procedures.

Establish and maintain effective working relationships with clients and their families, other members of staff, outside agencies and institutions, and the general public Plan and organize work and prepare adequate records and reports.

Communicate effectively with individuals on all levels and particularly with the aged.

Capacity for on-the-job learning and growth in skill levels affecting delivery of services. Must be an active participant in the facilitys Quality Assurance/Performance

Improvement (QAPI) program and Performance Improvement Project (PIP) and carry out roles and responsibilities as assigned.Must attend trainings for QAPI, follow policies and procedures in support of QAPI efforts, and support QAPI initiatives and findings.Must also report potential or actual non-compliance issues to the facilitys QAPI committee.

Perform other reasonable duties as assigned by supervisor.

Other reasonable duties as assigned by supervisor.Additional duties may be required, although they may be assigned to another employee due to special circumstances applicable to the employee
