Anchor Health Home Care is Hiring a Client Care Coordinator Near Brooklyn, NY

Role Overview:

As a Client Care Coordinator, you will represent Anchor Health by communicating with patients and assisting them with daily challenges. The ideal candidate will be warm, energetic, compassionate, and capable of thriving in high-pressure environments. This role also involves visiting each client once every three months, with an average caseload of approximately 30 clients.

Qualifications and Experience:

  • Exceptional customer service skills with strong verbal and written communication abilities.
  • Previous experience as a Service Coordinator or Case Manager advantageous
  • Detail-oriented approach to tasks.
  • Proficiency with Microsoft Word/Excel and willingness to learn scheduling and training software systems.
  • Ability to prioritize tasks and manage multiple responsibilities efficiently.
  • Maintain professionalism, composure, and positivity under pressure.
  • Project management skills, including establishing objectives, milestones, and creating presentations.
  • Capability to work independently while also being collaborative.
  • Healthcare experience is preferred; homecare experience is advantageous.
  • Bilingual proficiency in English and Spanish is preferred.
  • Bachelor's degree strongly preferred but not required.

Job Type: Full-time

Pay: $22.00 - $28.00 per hour

Expected hours: 40 per week
