**Job Summary:**
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, collaborating with the hospice team to provide patient care.
**Responsibilities:**
-Hospice case management and admissions
- Conduct patient assessments and develop individualized care plans
- Coordinate discharge planning and ensure smooth transitions for patients
- Collaborate with the IDG team to deliver comprehensive patient care
- Maintain accurate medical records and ensure compliance with HIPAA regulations
- Work in SNF, ALF/BC, and home settings
**Qualifications:**
- Valid RN license
- Proven experience in Hospice case management
- Strong background in hospice care
- Proficient in admission and patient assessment techniques
- Knowledge of hospice care principles and experience working in hospice environments
- Familiarity with HIPAA regulations and adept at maintaining medical records accurately
