Homelessness Prevention Case Manager to support families at-risk of homelessness through financial assistance, temporary case management, and connections to resources and support in the community. This individual must be a self-starter, highly organized and detail oriented with strong communication skills, experience in case management, understanding of and sensitivity to the needs of low-income families, and ability to work flexibly and creatively and have a passion for our mission – to change lives and impact poverty. POSITIONSUMMARY

The HomelessnessPrevention Case Manager workswith low-incomefamilies to preventorend homelessness by focusing on one-time and short-term financial assistance, strength-based case management, advocacy, landlord mediation,outreachandothersupportiveservicesthatresultinmaintainingstable,permanenthousing.TheCase Manager screens, assesses, and conducts intakes for prospective participants.TheCaseManagerprovidesenrolledfamilieswithflexiblefinancialassistance,temporarycasemanagement,and other support that takes a creative “whatever it takes” approach to ensure that families maintain permanent housing. This position is part of the Family Assistance Team and reports to the Family Assistance Manager.

RESPONSIBILITIESANDDUTIES

CaseManagement(75-85%)

Conductsprescreensandintakestodetermineparticipanteligibilityandassessesappropriatelevelof assistance needed.

Engagesfamiliesthroughassessmentofstrengthsandneedsanddevelopmentofgoalsandserviceplan to ensure families remain stably housed.

Maintainsaflexiblecaseloadofupto30ormorehouseholds,includinghouseholdsreceivingone-time financialassistanceandhouseholdsparticipatingincasemanagement.Workswitheachcasemanaged family to develop and implement a strength-based case management planto overcome economicand housing challenges and maintain stable housing.

Providesandassistshouseholdswithinformation,referrals,andconnectionstodevelopasupportsystem, including connecting with public benefits and other community resources to maintain stable housing.

Actsasanadvocateinmaintainingstablehousing,includingassistingfamilieswitheducationontenant’s rights,landlordmediationandadvocacy,andconnectionstolegalservices,asneeded.

Assistsfamilieswithfindingandsecuringnewhousing,whenneeded.

Supportsfamilieswithbudgetcounselingandfinancialliteracyservices.

Assistsfamilieswithimmediatecrisisintervention,advocacy,problem-solvingandotherappropriate interventions, as needed.

Connects families to other opportunities and resources at SHCS, including essential services,self- sufficiencyprograms,andopportunitiestovolunteerandtoparticipateinorganizingcommittees.

Communicates with households at regular intervals after they have received financial assistance or completedcasemanagementtoassesshousingstatusandprovideadditionalresourcesorsupportwhen needed.

Develops,tracks,andanalyzesmonthlygoalsandobjectivesinaccordancewiththeoperatingplanand funder requirements.

Documentsprogramactivity,includingprescreens,intakes,financialassistance,andcasemanagement into HMIS and Salesforce databases within one to three business days.

Maintainscasefilesandprogressnotes,adheringtoprofessionalstandardsanddesignatedtimeframes.

Implementsprogramevaluationtools(i.e.,pre/posttests,satisfactionsurveys,etc.),analyzesandreports on outcomes.

Submitsmonthlynarrative,data,andotherreportsasrequired.

Processesfinancialassistancepayments,asneeded.

ParticipationandLeadership(10-20%)

Engagesanddevelopsprogramvolunteersbyprovidingeducation,training,feedback,andsupport.

Supportstherecruitment,engagement,anddevelopmentofleadersthrough1:1meetings,training,and committee support.

SupportsthedevelopmentandongoingworkoftheFamilyAssistanceAdvisoryCommittee.

WorkscollaborativelywiththeFamilyAssistanceTeamtoensureappropriatecoverageofallteam responsibilities, including providing backup staff support for other program activities.

Supportscontinuouslearning,evaluation,andimprovementwithintheFamilyAssistanceTeam.

Supportstheplanningandparticipatesinagency-wideprograms,events,andoutreachefforts.

Supportstheplanningandattendsstaffmeetings,retreats,andotherteamandagencyevents.

Performsotherdutiesasassigned.

OutreachandEngagement(5%)

ParticipatesineffortstopromotetheavailabilityofhomelessnesspreventionassistancethroughSHCSandits collaborative partners, including publicly representing the program and organization with community partners and at community fairs or other events.

Developsandnurturescollaborativerelationshipswithcommunitypartners,includingschoolsandthecourt system, among others.

REQUIREDQUALIFICATIONSANDEXPERIENCE:

Strongcommitmenttoourvisionofacommunityunitedtoensurethateverychildandadultisfreefrom poverty.

Threeyearsofexperienceinhumanservicesorrelatedfield.

BilingualandbiculturalinEnglish-Spanishwrittenandverbal.

Experienceprovidingculturallycompetent,strength-basedcasemanagement,supportiveservices,and linkages to community resources.

Experience and knowledge in the functions of intake, assessment, service planning, case coordination, caseconferencing,serviceplanimplementation,crisisintervention,monitoringandfollow-up,andcase closure.

Strongcommunicationskills,includingtheabilitytodeliverdifficultmessageswithcompassionandto communicate respectfully with families in stressful situations.

Sensitivitytoissuessurroundinghouseholdsexperiencinghomelessness,substanceuse,mentalillness,or disabilities.

Abilitytothriveinaflexible,fast-pacedenvironment,whilemaintainingapositive,solution-oriented approach.

Abilitytoworkcollaborativelyinateamenvironmentandtoworkindependentlywithlimitedsupervision when necessary.

ComputerskillsincludingproficiencyinMSWord,Excel,PowerPoint,GoogleSuiteandabilitytoquickly learn and develop proficiency with tracking database systems.

ValidCAdriver’slicense,reliablepersonalvehicle,andvehicleinsurance.

Successfully completeabackgroundcheck.

PREFERREDQUALIFICATIONSANDEXPERIENCE:

Bachelor’sdegreeinsocialwork,publichealth,orrelatedfieldorequivalentexperience.

Twoyearsofexperienceworkinginoutreach,shelter,orsupportivehousingprogramsforhomelessorat- risk households.

Experienceworkingwithlandlords,localrentalmarkets,and/orHousingFirstfocusednon-profits.

REPORTINGRELATIONSHIP:

TheHomelessnessPreventionCaseManagerreportstotheFamilyAssistanceManager.

Positionisnon-exempt,full-time, temporary,andreportstotheFamilyAssistanceManager. COMPENSATION

Thispositionisanon-exempt,hourlypositionwiththestartingrateof$25-$27perhour.Thesalaryis

commensurate with experience.
