Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for coordinating and providing comprehensive care to patients, ensuring their needs are met and that they receive the highest level of support.
Responsibilities:
- Conduct thorough assessments of patients' needs, including physical, emotional, and social factors
- Develop and implement individualized care plans based on assessment findings
- Collaborate with healthcare professionals to coordinate and facilitate patient care
- Provide patient education on disease management, treatment options, and available resources
- Assist with discharge planning and ensure a smooth transition from hospital to home or other care facilities
- Maintain accurate and up-to-date medical records and documentation
- Communicate with patients, families, and caregivers to address concerns and provide support
- Advocate for patients' rights and ensure they receive appropriate services and resources
- Collaborate with interdisciplinary teams to develop strategies for improving patient outcomes
Skills:
- Strong knowledge of medical terminology and diagnostic evaluation processes
- Experience in case management or related field preferred
- Ability to effectively communicate with patients, families, and healthcare professionals
- Proficient in using electronic medical record systems
- Excellent organizational skills with the ability to prioritize tasks and manage time effectively
- Knowledge of discharge planning processes and resources available for patients' post-hospitalization needs
- Familiarity with primary care principles and practices
- Experience working with pediatric patients is a plus
