Since 1985, Albuquerque Health Care for the Homeless has dedicated its mission exclusively to providing services to people experiencing homelessness. AHCH provides a distinctive continuum of integrated care through outreach and site-based services to address the health-related causes and consequences of homelessness. We are currently looking for an individual to join our team as a Referral Specialist. The role of the Referral Specialist supports the Primary Care Clinic and Patient Access Representatives with clerical/administrative duties. This position coordinates all external and some internal patient services, including processing of referrals and aiding patients in scheduling external consultations, diagnostic testing, and surgical procedures.

Duties and Responsibilities:

• Process referrals and authorizations and then input into a data bank
• Coordinate follow-up care with the referral physicians as approved by an AHCH provider
• Ensure that all necessary medical records are forwarded to referral provider
• Act as a resource for staff and patients with questions regarding referral policies and procedures
• Enroll or assist patients in enrolling in public benefits programs
• Greet patients/clients and provide information as needed
• Answer phone calls and respond to emails/faxes as appropriate and needed
• Work as a member of a patient care team and contributes suggestions for improving unit and organization operations
• Determine eligibility and process applications for financial assistance, including Medicaid and Medicare
• Attend primary care clinic meetings/huddles
• Other duties as assigned

Minimum Qualifications:

• 1 year of experience working in a Referral Specialist, Medical Front Desk, or Medical Assistant role
• Ability to work in fast-paced, stressful/noisy environment
• Excellent attendance history
• High attention to detail and organization
• Ability to proficiently use technology in a medical or dental setting
• Ability to work and communicate with a diverse range of clients/patients and co-workers

• High school diploma or equivalent

Preferred Qualifications:

• Previous experience working in a Federally Qualified Health Center (FQHC)
• Experience working with underserved or vulnerable populations
• Bilingual or Spanish speaking
• Experience with Electronic Heath Record (EHR)

• Bachelor’s degree in Health Administration
