Aging Services of North Central Massachusetts is Hiring a Bilingual Case Manager Near Leominster, MA

Description

POSITION SUMMARY: A Case Manager conducts comprehensive in-home assessments with elders, helps determine needs and eligibility for government subsidized services, creates care plans and coordinates services. Duties include data entry into computer database, communication and consultation with family members, other Aging Services of North Central Massachusetts staff and community professionals.

Bilingual language preferred isSpanish speaking. The responsibilities would include completing all of the following on behalf of Spanish-speaking elders: 

  • Completing face to face client-oriented needs assessments that identify issues or problems which inhibit independent living,
  • Determining financial, functional, and program eligibility for a variety of service and program options: SHC, Respite, ECOP, CHOICES, MA Waiver, 300% and Spousal Waiver. 
  • Identifying and referring applicants appropriate for AFC, GAFC, PCA, Caring Homes, Supportive and Congregate Housing, Caregiver Initiatives, Money Management, Volunteer Services, etc.
  • Participating in interdisciplinary work with ASAP RNs. Functioning as a team partner in the Comprehensive Service and Screening model, completing on-site assessments and case conferences at nursing homes to assist individuals to return to the community.
  • Mobilizing and supporting informal supports, 
  • Authorizing and monitoring of services, 
  • Coordinating with other community agencies and services,
  • Providing information, referral and follow up,
  • Advocating effectively on behalf of elder applicants and clients to assist them in availing themselves of wide range of public and private benefits and programs.
  • Assisting in problem resolution.
  • Maintaining a thorough knowledge base regarding community resources,
  • Participating in staff meetings and in service trainings,
  • Maintaining case record documentation in a timely and comprehensive manner in accordance with EOEA, DMA regulations, standards and ASNCM procedure manuals
  • Providing supportive counseling to clients and to family members, when appropriate.
  • Serving on after-hours on-call team to respond to consumer needs.
  • All Case Managers shall adhere to the principles of client self-determination and individual choice, involving clients in the identification of problems and design of appropriate service plans. All Case Management activities shall be designed to promote optimal independence among clients.
  • The Case Manager shall perform other related duties as assigned.

QUALIFICATIONS AND EDUCATION REQUIREMENTS:

Must be fluent in Spanish and English. A Bachelor’s degree in social work or a related field and two years’ experience in casework, or related activity. Experience in a human service field is preferred. A Master’s Degree in Social Work or a related field and one year of related experience is preferred.

The Worker must be able to work with a diverse population, make mature judgements, possess good communication skills and maintain effective interpersonal relationships. Knowledge of basic medical, mental health, social service delivery systems, as well as acceptable record keeping practices is essential.

A valid driver's license and a legally insured and registered motor vehicle are required. 

PHYSICAL:

  • Visual, speaking, auditory and mobile capacity necessary:
  • Capacity to see computer screen, read written material, and drive a car.
  • Capacity to hear and speak on the telephone.
  • Capacity to communicate verbally with consumers, caregivers, supervisors and managers.
  • Capacity for fine manipulation in the frequent use of office equipment such as computers, copy machines, fax machines, telephones, calculators, etc.
  • Capacity to drive in all-weather to visit consumers in their homes
  • Capacity to navigate uneven terrain in all-weather to visit consumers in their homes
  • Capacity to climb stairs
  • Ability to sit or stand for extended periods of time.
  • Occasional reaching and grabbing objects with both hands, twisting of hand and wrist, and pushing and pulling of objects.
  • Occasional bending, squatting, and twisting to perform work functions.
  • Occasional capacity to lift up to 25 pounds.

MENTAL: 

  • Ability to understand and/or interpret complicated program instructions and laws.
  • Capacity to learn complicated computer software
  • Capacity to deal rationally and calmly with varying personalities
  • Capacity to work well in fast paced, rapidly changing environment. 

ENVIRONMENTAL:

  • Work is split between indoor office work and traveling outdoors to meetings, trainings and consumers’ homes.
  • Must be able to tolerate odors and pollutants including but not limited to smoke, air fresheners, pet dander, personal odors, etc. from a variety of in-home conditions.
  • Must be able to tolerate by-products of office machine operation.
  • Must be able to tolerate heat and cold of seasonal changes and indoor temperatures.

Compensation and Benefits:

This is Monday-Friday, 35 hour-per-week position with competitive pay, mileage reimbursement, excellent work environment and an outstanding benefits package, including health, dental, vision, company paid life insurance, and health reimbursement account. Additionally, this position offers a retirement plan, generous paid time off, and employee referral bonuses. Upon Hire you will receive 6 days of personal time and accrue vacation immediately. $1,000 hiring bonus is payable in two payments of $500 upon successful completion of 3 and 6 month orientation periods.
