Description:

• Working at the Covington location at the Med Surg hospital.
• Supporting the in-patient and ED patient populations
• Mostly working with adults, but occasionally some teenagers will come in.
• Working with psychiatric/mental health patients in their in-patient ward.
• Need to do mental health evaluations
• Cross continuum care coordination (clinic, provider care, urgent care, phone care, skilled care, etc)
• Support with discharge planning
• Psychosocial assessments and psychosocial interventions
• Identify opportunities to continually improve patience care, services, and population health

Additional Skills & Qualifications:

• Need to be a clinical social worker. They can accept either a associates or an independent license. If associates degree need to commit to obtain their LICSW within 4 years.
• Acute care experience
• 2-3 Hospital experience
• Need to have proficient computer skills
• Exposure to an EMR system is preferred ideally EPIC (what Multicare uses)
• Ability to have organizational skills (managing the work and flow).
• MSW degree
