Social Worker MSW - PACE/Elderplace This position will be traveling to see patients in the King county area.

Job Description:

Social Worker MSW - As a member of the interdisciplinary team, provides social work services to participants in a manner which is consistent with the mission and core values of Providence Health System. Spends a majority of time in direct service activities within the Center, including one-on-one and group counseling, interaction with other team members, and ongoing assistance to participants to address social issues. Based on comprehensive assessments, provides perspective on the social, emotional, cognitive and ethnic factors that impact the creation of an individualized integrated plan of care. Accesses community and financial resources for participants and ensures continuity of care across the continuum. Conducts social work assessment and intervention in other care settings, including the participant's home, as needed.

Required Qualifications:

• Master's Degree in Social Work
• Washington Advanced Social Worker License upon hire or
• Washington Advanced Social Worker Associate License upon hire or
• Washington Clinical Independent Social Worker License upon hire or
• Washington Clinical Independent Social Worker Associate License upon hire
• 1 year work experience with geriatric population
• 2 years social work with geriatric age groups from diverse ethnic cultures

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life. Our service lines include assisted living/skilled nursing/rehabilitation, home health, home infusion/pharmacy services, home medical equipment, hospice and palliative care, Program of All-Inclusive Care for the Elderly (PACE), personal home services/private duty care and supportive housing.
