Acacia Network is Hiring a Case Manager Supervisor (Stabilization Unit) Near Brooklyn, NY

The Case Manager Supervisor will support and engage clients in all aspects of social services including case management, permanent housing placement, employment, entitlements, recreation, etc. The Case Manager Supervisor works with community organizations, government agencies, and property owners to secure living arrangements, negotiate rents, deposits, and move-in terms on behalf of their clients. Case Manager Supervisor will connect clients to needed services, ensure all applicable entitlements are secured, and support and progress clients towards independent living.

The salary rate for the Case Manager Supervisor is $50,000 - $55,000 annually based on relevant experience.

KEY ESSENTIAL RESPONSIBILITIES:

  • Conduct comprehensive client assessments to collect functional, environmental, psycho-social, educational, and health information as appropriate to develop and create an effective care plan.
  • Schedule and conduct weekly/biweekly meetings with clients.
  • Create Independent Living Plans and monitor plans for effectiveness.
  • Ensure clients have appropriate medical insurance and are referred to medical providers.
  • Maintain updated case files with necessary documentation; document all efforts and services provided to clients.
  • Develop support systems to meet client needs by identifying and coordinating a variety of available services.
  • Evaluate client risk and assess the need for immediate intervention.
  • Interpret and explain DHS regulations and service programs to the client.
  • Provide supportive counseling and advocacy for clients.
  • Determine quality and effectiveness of services provided and if necessary, develop and implement new processes and procedures.
  • Build and maintain positive working relationships with staff and clients.
  • Engage clients in the problem-solving process.
  • Analyze situations and determine the proper course of action by making decisions and utilizing independent judgment.
  • Continuously know signs and symptoms of mental illness; understand cultural influences on behavior; identify the need for services; problem solves to assess the needs of clients; observe interactions for assessment purposes; explain regulations and findings to clients.

Permanent Housing:

  • Assess clients for housing to determine the best Exit Strategy.
  • Assess clients for all housing subsidies for eligibility and assist in securing housing vouchers.
  • Assist clients in finding suitable housing.
  • Develop and maintain liaisons with landlords and the community.
  • Help clients with all housing applications.
  • Assist clients through the entire housing process from the application process to meeting prospective landlords, lease signing, and moving out.
  • Work directly with realtors, landlords, and Social Services Department to expedite the lease signing process.
  • Inspect apartments/rooms, and check the status of the Public Assistance Case.
  • Achieve the monthly move-out target assigned.
  • Prepare monthly housing status reports, and update housing notes every week.
  • Document all housing efforts.

Employment/Recreation:

  • Assist clients in finding suitable employment.
  • Develop and maintain liaison with employers and the community.
  • Assist clients in securing job interviews.
  • Prepare clients for job interviews.
  • Conduct employment-related workshops – resume, interviewing, dressing for success, etc.
  • Meet employment-related targets.
  • Document all employment efforts.
  • Organize recreational activities onsite and identify external opportunities.
  • Maintain records and prepare employment/recreation-related reports.
  • Develop, prepare, and present training and educational programs/workshops.

Other:

  • Participate in and facilitate interdisciplinary team meetings to discuss and overcome resident barriers.
  • Attend meetings, training, and workshops as required.
  • Other duties in keeping with the scope and nature of the position.

EDUCATION AND REQUIREMENTS:

  • 3 years previous relevant experience in Social Services required
  • Associate's degree required (Bachelor’s degree preferred)
  • Ability to network with landlords, real estate management companies, and community-based and city agencies to identify permanent housing opportunities for shelter clients
  • Ability to use business acumen to exercise good judgment and decision-making
  • Ability to deal with sensitive issues with diplomacy and discretion
  • Must be able to multi-task with strong organizational skills; must be a resourceful self-starter
  • Excellent interpersonal skills and ability to communicate both verbally and in written form
  • Commitment to cultural diversity and sensitivity
