We are looking for an initial part time or full time RN to work with a new Comprehensive home care agency doing HOUSING WITH SERVICES (HWS) in Minneapolis and St Paul. The responsibilities include prospective client assessment to determine if this would be an appropriate setting. There are 4 homes built 2018 each having room for 5 clients. apply online. Responsibilities:
- Initial assessment and setting up the care plan
- Coordination with PMP and case managers
- Ongoing reassessments
- Medication management
- Staff orientation and training
- Staff supervision
