Substance Use Disorder Professional (SUDP)

Salary: $32.00 – $45.05 per hour, payable bi-weekly. The salary scale for this position is between steps 0 and 19, depending on experience. This is an exempt salary position, 40 hours a week. Applicants must pass an extensive background check.

About Us
Country Doctor Community Health Centers (CDCHC) is a not-for-profit organization that operates two primary care clinics, an After-Hours Clinic, two School-Based Health Centers, a low barrier Medication-assisted treatment clinic, and a Dental Clinic. Our Carolyn Downs Family Medical Center was founded in 1970 by the Seattle Black Panther Party in Seattle’s Central District. Our Country Doctor Community Clinic was established in 1971 by residents of the Capitol Hill neighborhood in conjunction with the Seattle Liberation Front. The primary goal of each clinic was to provide low-barrier, high-quality healthcare services and promote social justice.

What You’ll Be Doing

As a Substance Use Disorder Professional, you are a vital member of our Behavioral Health team, responsible for delivering exceptional substance use treatment services to our patients in an outpatient setting. With your extensive knowledge and expertise, you have the opportunity to make a positive impact on the lives of those suffering from substance use disorders by providing personalized assessments, diagnoses, counseling, and treatment. By working collaboratively with our staff, you can also help to provide consultation and training on working with patients with substance use disorders, making a lasting difference in our community.

DUTIES AND RESPONSIBILITIES

  • Serve as a contact for medical providers to discuss patient issues or concerns related to possible substance use disorder symptoms
  • Perform Substance Use Disorder assessments and make treatment recommendations
  • Provide care coordination and case management services, with a focus on rapid placement into the least restrictive and appropriate level of care
  • Develop relationships with community resources
  • Counsel patients, individually or in group sessions, to assist in pursuing recovery, adjusting to life, or making changes.
  • Complete and maintain accurate records or reports regarding the patients’ histories and progress, services provided, and other required information
  • Develop, review and evaluate patient treatment plans based on research, clinical experience, and patient histories
  • Provide patients or family members with information about substance use disorder issues and available services or programs, making appropriate referrals when necessary
  • Plan or implement follow-up or aftercare programs for patients to be discharged from treatment programs
  • Collaborate with staff at school-based sites to provide services to young people using substances, and to provide recovery-oriented support to the wider school community
  • Counsel patients/support system on harm reduction methods
  • Attend Addiction Medicine Program (AMP) meetings and coordinate with program

ABILITIES

  • Proficient in Microsoft Office and working on PCs
  • Comfortable working within electronic medical record systems
  • Treat patients in accordance with the mission and philosophy of the organization
  • Work independently and collaboratively with a clinical team in a medical setting

You’ll need these qualifications.

  • You have an Active Substance Use Disorder Professional License in Washington State.
  • You have a minimum of 2 years of experience required
  • You have experience and background in harm reduction philosophy and treatment.
  • You have experience working with adolescents.
  • You have a valid driver's license.
  • Active CPR Certification
  • You have experience providing care to medically underserved populations.
  • You interact and work well with people from diverse cultures and backgrounds.
  • You’re comfortable working with Microsoft Office, Google products, and computers.

It would be nice if

  • You bring experience with Epic electronic medical records (EMR) software.
