Salveo Integrative Health is looking for one full-time and one PRN Discharge Planner for an inpatient mental health facility in Athens, GA. 12 hour shifts, 9a-9p. Please inquire within.

Job Overview:
We are seeking a Planner to join our team in the healthcare industry. As a Planner, you will be responsible for coordinating and managing the discharge planning process for patients in an inpatient behavioral health setting. This is a vital role that ensures patients receive appropriate care and support during their transition from the facility to their next level of care.

Responsibilities:
- Collaborate with healthcare professionals to develop and implement individualized discharge plans for patients
- Conduct assessments to identify patients' needs and create plans that address their medical, social, and emotional requirements
- Coordinate with insurance providers, community resources, and other healthcare facilities to arrange necessary services for patients after discharge
- Educate patients and their families on post-discharge care instructions, medication management, and available support services
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information
- Maintain accurate and up-to-date medical records related to discharge planning activities

Experience:
- Previous experience in discharge planning or case management is preferred
- Familiarity with medical terminology, ICD-9 coding, and medical records management
- Strong communication skills to effectively interact with patients, families, and healthcare professionals
- Excellent organizational skills to manage multiple tasks and prioritize workload
- Ability to work independently as well as part of a multidisciplinary team

-Bachelor's degree required, Master's degree preferred
