Description
Mt. Macrina Manor is a 133 bed, not for profit, skilled nursing facility that has served Southwestern Pennsylvania since 1971. The grounds are located approximately one hour south of Pittsburgh in a scenic, peaceful rural area directly off Route 40 in Uniontown. The facility offers 24 hour nursing care, rehabilitative services, memory care, and pastoral care to all denominations. We accept all residents without regard to race, color, age, sex, national origin, religious creed, disability or genetic information. Our mission is based upon the sponsorship of the Sisters of St. Basil the Great. Our staff continues the mission of the Sisters of providing joyous, compassionate care that preserves the human dignity of all that we serve.
PURPOSE AND SCOPE:
The RNAC assists with accurate and timely completion of both the RAI and Care Management process from admission to discharge in accordance with Mt. Macrina Manor policy and procedures, state and federal guidelines, and all other entities as appropriate.
D UTIES AND RESPONSIBILITIES:
  1. Demonstrate an understanding and puts into practice person-centered philosophy by entering into a relationship with the residents, families and staff where the goal is to provide a comfortable and home-like environment and the relationship develops into a meaningful and satisfying one for all involved.
  2. Works in collaboration with the IDT to assess the needs and formulate a plan of care for the residents.
  3. Completes all required sections of the MDS as per facility policy and verifies with other IDT that they have completed MDS assessments to ensure timely completion. Complies with all federal and state regulations, and authorities regarding completion and coordination of the RAI process.
  4. Completes accurate coding of the MDS with information obtained via medical record review, outside medical records, as well as observation and interview with direct care staff, resident, family members, and responsible parties.
  5. Monitors MDS and care plan documentation for residents. Ensures documentation is present in the medical record to support MDS coding.
  6. Identifies and evaluates need for Significant Change Assessments, places the significant change to the calendar, makes IDT aware that a significant change assessment is scheduled and needs completed. Email the Admission group notifying of the significant change.
  7. Communicates need for setting ARDs to capture relevant data that should be reflected on the MDS.
  8. Maintains current MDS status of assigned residents according to state and federal guidelines.
  9. Responsible to provide education and ensure staff is documenting accurately the care provided to maximize reimbursement within regulatory guidelines.
  10. Obtains and ensures Skilled Certification/Re-Certification documentation is accurate and signed timely by the Physician/Physician Assistant/ Nurse Practitioner according to state and federal guidelines.
  11. Facilitate care planning process in relation to MDS through being the leader of the care plan meeting for all residents in the community, ensuring accuracy, completeness and interdisciplinary integrity. Completes and updates care plans for all residents accurately and timely. Communicates all identified resident concerns/needs to the RN Unit Manager. Update POLST with care conference and communicate when updated.
  12. At the direction of Nursing Administration, attends IDT meetings and other meetings in order to gather information, communicate changes, and maintain and update records.
  13. Continually updating knowledge base related to data entry and computer technology.
  14. Generate reports required by Nursing Administration.
  15. Case Management responsibilities to include but not limited to the following: Clinical updates to required insurances that are accurate and timely, discharge information to required insurances, issuance of NOMNC when required for Managed Care residents, develop effective working relationships and cooperate with IDT throughout the entire case management process, keep update/review date calendar accurate, review weekly/daily PPS sheets to ensure accurate information to the required insurance. Attends PPS meeting as assigned.
  16. Participates on various facility committees as assigned (ie. Safety, wellness, etc.)
  17. Assist as needed with all Department of Health and Life Safety surveys and plans of correction.
  18. Assist in preparing and completing scheduled UMR audits.
  19. Assist in gathering information to respond to various audits such as RAC audits or any other type.
  20. Keep current with ICD 10 information and changes to ensure resident diagnosis codes are accurate and appropriate.
  21. Maintains hours as defined by Nursing Administration in conjunction with case management responsibilities to ensure the best outcome for the resident.
  22. Maintain appropriate licensure.
  23. Customer Service : Demonstrates an understanding and puts into practice excellent customer service by creating a positive first impression, handling with care, being a team player, communicating professionally and acting like an owner.
  24. Compliance : Demonstrates commitment to operating in accordance with Mt. Macrina Manor's mission, vision, and values. Specifically this means maintaining accuracy and integrity in all of its activities, including providing quality, timely, and necessary care to its residents and operating within the limits of local, state and federal regulations.
  25. HIPAA : Demonstrates confidentiality through a conscious effort to keep all personal information private. This may include the resident's identity, physical or psychological condition, emotional status, or financial situation.
  26. Actively participates in the Resident Dining Program after completing the Feeding Assistant Training Program unless already licensed or certified in an appropriate field. Functions may include: Transporting residents to dining areas, Passing meal trays and assisting residents with tray set-up, Assisting with feeding residents who are not considered high risk, Adhering to dining schedules and assignments, and Completes tasks within safety and regulatory guidelines.
  27. Any and all other duties, as assigned.
QUALIFICATIONS AND REQUIREMENTS:
  • Licensed as a Registered Nurse in Pennsylvania.
  • 2-3 years of experience in Long Term Care Setting.
  • 2-3 years of experience in completing MDS and facilitating interdisciplinary care planning.
  • Demonstrates good leadership skills and possesses the ability to motivate employees.
  • Organization skills required for timely completion of assigned tasks and projects.
  • Ability to complete tasks independently and within a group environment.
  • Proficient in communication and listening skills.
  • Possess a positive attitude to help promote a healthy work environment.
  • Proficient computer skills (Microsoft Word, Excel, Outlook, Point Click Care (PCC), Answers on Demand (AOD), eHealth Data Solutions, etc.).
  • Working knowledge of current Federal, state, and local regulatory requirements.
  • Ability to read, analyze, and interpret general information in the English language.
  • Ability to walk, lift, bend at knees and waist, stoop, kneel, reach, climb step stool, and stand in place for extended periods.
  • Ability to lift and/or move objects up to 25 pounds.
  • Ability to push or pull up to 100 pounds occasionally.
  • Ability to use close vision, peripheral vision, and adjust focus.
Due to regulatory requirement by the Centers of Medicare and Medicaid services, a COVID-19 vaccination is required and considered a condition of employment. To be eligible for hire with Mt. Macrina Manor, prospective employees must be fully vaccinated at time of orientation. A person is considered fully vaccinated two weeks after their second dose in a two-shot series, such as the Pfizer-BioNTech or Moderna vaccines, or two weeks after a single-dose vaccine, such as the J&J/Janssen vaccine.
