The RNAC will be responsible for coordinating and conducting assessments of residents' health needs, ensuring compliance with state and federal regulations, and collaborating with interdisciplinary teams to develop individualized care plans. Your Responsibilities:

  • Conduct comprehensive assessments of residents' health status upon admission and at regular intervals according to facility policies and regulatory requirements.
  • Coordinate and complete the Minimum Data Set (MDS) assessments accurately and timely for each resident.
  • Collaborate with the interdisciplinary team, including physicians, therapists, and social workers, to develop individualized care plans based on assessment findings.
  • Monitor and evaluate residents' responses to interventions and update care plans as needed.
  • Ensure compliance with state and federal regulations related to assessment and care planning processes.
  • Provide education and training to staff members on assessment procedures, documentation requirements, and regulatory standards.
  • Serve as a resource for staff members regarding resident care needs and regulatory requirements.
  • Participate in quality improvement initiatives and assist in identifying areas for improvement in resident care processes.
  • Maintain accurate and complete documentation of assessments, care plans, and interventions in residents' medical records.
  • Communicate effectively with residents, families, and other members of the healthcare team regarding residents' care needs and goals.

  • Current Registered Nurse (RN) license in the state of Pennsylvania.
  • Minimum of 2 years of experience working as a Registered Nurse in a skilled nursing facility or similar setting.
  • Certification in Resident Assessment Coordinator (RAC-CT) preferred.
  • Strong assessment skills and ability to collect and analyze data effectively.
  • Excellent interpersonal and communication skills.
  • Knowledge of state and federal regulations governing long-term care facilities.
  • Ability to work collaboratively as part of an interdisciplinary team.
  • Proficiency in electronic medical record (EMR) systems and Microsoft Office applications.
  • Commitment to providing compassionate and high-quality care to residents.
  • Flexibility to adapt to changing priorities and work schedules.
