Nurse Practitioner - Adult Geriatric Population - LIFE/PACE Program

Our client is seeking a Nurse Practitioner to join their team in the Little Silver, NJ area.

Schedule:

• Full Time: Monday-Friday, 8-4:30 PM, no weekends or nights
• On-call once every 6-8 weeks and can be done from home/telephonically

Compensation: $115,000 - $130,000/yr, dependent on experience and hours worked

Nurse Practitioner Responsibilities:

• Perform comprehensive physical evaluations for a Geriatric population (~12 patients per day)
• Develop individualized care plans and ensure appropriate follow up through the care continuum
• Work collaboratively with an interdisciplinary team consisting of Physicians, RN’s, Dieticians, Pharmacists, Psychiatrists, Social Workers, Cardiologists and Podiatrists
• Prescribe all necessary medications/treatments and educate patients/families about medication usage

Qualifications:

• Master’s Degree in Nursing
• Licensed Family or Adult Geriatric Nurse Practitioner in the state of New Jersey
• 1 year of experience as a Nurse Practitioner with the Adult Geriatric population
