ECM Family Resource Specialist- This is a full-time position, Monday-Friday from 9:30am-6:00pm, hybrid-three days in the office and two days remote. This position is located in our San Francisco Bay View location.
Program Summary
Enhanced Care Management (ECM), an initiative of California Advancing and Innovating Medi-Cal (Cal AIM), is a whole-person, interdisciplinary approach to care that addresses critical but non-clinical needs (vocational, housing, basic needs, educational, legal, etc.), to improve the quality of life and health outcomes of Medi-Cal Members. Services are community based, interdisciplinary, high touch and person centered.
Position Summary
The Family Resource Specialist for Enhanced Care Management (ECM) supports the non-clinical needs of parents/caregivers of children who are enrolled in the San Francisco Health Plan through a process of care coordination. The Family Resource Specialist, stationed within the Edgewood Family Resource Center, outreaches, and engages families to enroll them in ECM services, completes a comprehensive needs/strengths assessment and service plan, provides resource information and brokerage to address unmet needs, assists families in accessing and navigating through resources, monitors service plan progression, and provides follow-up and aftercare if needed. Services are provided in the office, in family homes, in other community locations, and through phone/text/email, or video conferencing.
Essential Functions
- Be an Edgewood Ambassador. Outreach to the agency and the community to provide information about the benefits of ECM services, the eligibility requirements, and the enrollment process. Ensure a welcoming and seamless service provision for all families and referents.
- Be a Care Coordinator and Family Advocate. Provide case management support that includes consistent outreach and engagement to families; assist families in developing the skills and knowledge to eventually access resources independently; arrange targeted support groups and educational workshops for families on various relevant topics; distribute in-kind donations; and provide coordination with other services important in a family’s life.
- Be a record keeper. Keep detailed case records in a timely and professional manner and in accordance with program and agency standards and deadlines, such as sign-in sheets, evaluations, enrollment forms, client satisfaction surveys, assessments, service plans, case notes, and incident reports. Document and record every service performed, whether it be with an individual, group, team, or organization.
Applied Knowledge, Skills, and Abilities
- High School Diploma or General Education Diploma is preferred.
- Two years of community, social, or public health work experience.
- Spanish-language speaking and writing fluency preferred.
- Must trustfully work autonomously in the field.
- Ability to use Microsoft Office programs.
Salary Information
This position pays an hourly rate range of $28.00- $30.80 depending on experience and bilingual language differentials. *Please note- the language differential is contingent on passing a language proficiency test.
