The Care Manager is responsible for coordinating and facilitating access to appropriate health care services for patients with complex and chronic diagnoses. Develop, coordinate, and implement, in conjunction with provider, care goals for patients with complex health care needs.


Essential Functions

  • Ensure identification and enrollment in care management services of patients with complex health care needs.
  • Work with Provider to develop, coordinate, and implement care goals for patients with complex health care needs. This includes making initial assessments, assisting with referrals to specialists, and providing information about community and health resources to families.
  • Participate in routine team meetings with each medical provider to proactively review care managed patients, upcoming appointments and provide feedback on important events.
  • Provide review of top 2% of tiered patients on no less than a quarterly basis.
  • Provide direct patient care and/or evaluation as needed and that is within one’s scope of their license and/or certification.
  • Track patient goals for care managed patients via appropriate EMR documentation. Utilize approved care plan templates. Create and maintain annual care plans for care management patients at no less than 75% completion.
  • Coordinate care with specialist offices and other care managers including community resources and IEP’s.
  • Conduct quarterly (or more) outreach to care managed patients and document each within the EMR system.
  • Serve as the contact point, advocate, and information resource for family and community partners. Provide education to families/patients and follow-up on specific conditions and care protocols as applicable within the scope of one’s license and/or certification.
  • Facilitate family access to Medical Home providers, staff, and resources.
  • Conduct follow-up with families of hospitalized patients and document appropriately within the EMR.
  • Research, catalog, and maintain resources to link families to appropriate educational, informational, and referral sources.
  • Promote Care Management program to families and others.
  • Successfully work in a fast-paced, changing environment with effective time management and ability to multitask and prioritize workload.
  • Always maintain confidentiality of sensitive patient information.
  • Maintain excellent patient relations.
  • Contribute to team effort by accomplishing related goals and results as needed.
  • Attend and actively participate in huddles and team meetings.
  • Display high standards of office conduct.
  • Punctual, regular, timely and dependable attendance.

Education and Experience

  • RN/LPN Candidates - current license in the State of Oregon required and degree from an accredited nursing program required.
  • Minimum of 3-5 years of experience in a family practice or pediatric clinical setting, preferably giving advice and performing triage
  • Current CPR certification required.
  • Care management experience preferred
  • Understand of clinic workflow required.
