The ICC at BAMSI provides support, the coordination of services and advocacy to parents/caregivers enrolled in the program. The Care Coordinator ensures the youths and familys voice is heard throughout the wraparound process and facilitates the provision of creative, customized services and supports to youth and families that enhance their lives and lead to positive outcomes.

The Community Service Agency (CSA) of the Brockton Area provides intensive coordination of care to support youth with serious emotional challenges as they remain in their home and community.

  • Bachelor's Degree Or Master's Degree and a minimum of 2 years of experience working with the target population pursuant to MCE credentialing criteria is required.
  • Must have a valid driver's license in state of residence and own means of transportation.
  • CANS, Wraparound knowledge, experience in navigating all child/family-servicing system, and experience advocating for family members who are involved in behavioral health systems preferred.

Responsibilities:

General Statement of Duties : The Care Coordinator provides support, the coordination of services and advocacy to parents/caregivers enrolled in the program. The Care Coordinator ensures the youths and familys voice is heard throughout the wraparound process and facilitates the provision of creative, customized services and supports to youth and families that enhance their lives and lead to positive outcomes.

Responsibilities:

  1. Conduct a comprehensive, home-based assessment inclusive of the CANS (Child and Adolescent Needs and Strengths) and Mental Status Exam, at a location of the familys choosing.
  2. Conduct Functional Behavioral Assessments and develop comprehensive safety plans with families.
  3. Identify and develop appropriate members of the Care Plan Family Team.
  4. Develop and implement a youth and family-centered Individualized Care Plan in collaboration with the family and its service team.
  5. Attend and actively participate in weekly individual and group clinical supervision to review progress, identify risk factors, improve communication, and foster professional growth.
  6. Refer and link families to appropriate supports as identified in the Individualized Care Plan including medical, educational, social, therapeutic, family-centered services and resources.
  7. Assist the family with strategies to navigate the mental health system and to identify and develop natural supports. Provide psycho-education, advocacy, support and transportation to families as needed
  8. Monitor, review, and update the Individualized Care Plan to reflect the changing needs of the youth and family in a timely manner and maintain overall responsibility for the clinical integrity of service planning and delivery.
  9. Complete the Child and Adolescent Needs and Strengths Assessment (CANS) quarterly and at discharge for each enrolled child. Participate in the rotation of the 24 hour on-call system and be available for telephone consultation to provide timely and effective crisis support services to youth and families and assist in effective intervention.
  10. Work with other BAMSI programs and the Mobile Crisis Intervention Team (MCI) to provide coordinated and comprehensive crisis diversion and intervention services.
  11. Maintain records, reports, and documentation for families in a timely manner, including insurance authorizations, Individual Care Plans, weekly progress notes, assessments and evaluation data. Conduct clinical practice in accordance with BAMSI policies, all state department regulations, Massachusetts Behavioral Health Partners (MBHP) regulations, Federal Medicaid Guidelines, the requirements of the Board of Registration in Medicine under the Patient Care Assessment Program, and any other regulatory agencies to which the program is subject.
  12. Completes Tier One National Wraparound Certification Requirements within 3 months of hire
  13. Meets weekly billing productivity expectations (a minimum of 30 hours per week for a full time employee.)
  14. Files 51As (Child Abuse and Neglect Report) with support from supervisor as required by law.
  15. Maintain regular and timely attendance, professional boundaries, quality clinical service and high professional standards at all times.
  16. Adhere to the agencys policy regarding confidentiality, and execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk.
  17. Complies with all state, federal and funding source policies, including policies on conflict resolution and mediation
  18. Perform other related work duties as needed or as assigned by supervisor or designee.
Qualifications:

EDUCATION/CREDENTIALS:

  • Bachelor or Master's degree and a minimum of two(2) years of experience working with the target population pursuant to MCE credentialing criteria required.
  • Certification in the Massachusetts Child and Adolescent Needs and Strengths (CANS) preferred.
  • Must have valid drivers license in state of residence and own means of transportation

EXPERIENCE:

  • Experience in navigating all of the child/family-serving systems and experience advocating for family members who are involved with behavioral health systems strongly preferred.

KNOWLEDGE/SKILLS/ABILITIES:

  • Working knowledge of the wraparound process and systems of care principles desired.
  • Must demonstrate sensitivity to cultural diversity and an ability to accept families, children and youth unconditionally without judgment or blame for the challenges they face.
  • Ability to communicate effectively with service providers and administrators, to work independently and as part of a team, and to embrace the system of care principles and family systems
  • Must possess excellent coordination, facilitation, mediation and crisis management skills.
  • Must possess excellent computer time management, and organizational skills.
  • Must be flexible and able to respond to families under stress and in crisis.
