Who we are looking for:

Are you the Intensive Care Coordinator we've been looking for? Our community service agency provides high fidelity wraparound services for Boston-based youth who struggle with emotional disturbance. We provide supportive services to youth under the age of 21 as they and their families identify and pursue their goals. In this role, you will facilitate a comprehensive care planning process that values the family's voice and preferences. You will actively engage other professionals in the fields of child welfare, special education, juvenile justice and mental health as you facilitate a collaborative care planning process on behalf of youth and their families.

JRI provides the training so that:

• you will be able to facilitate Care Planning Teams; coordinate multiple services/supports for MassHealth-enrolled children and youth under the age of 21 who struggle with emotional/behavioral health needs.
• you will serve as liaison with individuals, partnering agencies, and supportive programs or services to guide the care of youth.
• you will identify and assist families in accessing available services and promote the youth in successful engagement.
• you will provide culturally competent, trauma informed, wrap-around services that center the needs of the youth.
• you will support parents and caregivers in learning effective advocacy skills.

Requirements:

• Bachelor’s or Master’s Degree in Social Work or other related field required.
• Must have daily access to reliable, insured vehicle and active Driver’s License.
• Must be willing to maintain a flexible work schedule to accomodate client need.
• Experience navigating child/family-serving systems and working collaboratively with state agencies, consumer advocacy groups, and/or behavioral health facilities is preferred.
• Experience in working with and advocating for children/families with serious emotional/behavioral health needs is preferred.
• Knowledge of Wraparound systems of care and child welfare is preferred.
• Background and driving record checks will be performed.
