Job Description

Access Services is hiring an Administrative Case Manager in Bucks County, PA, with an office based in Warminster, PA. This role involves aiding children, young adults, and families who face difficulties in navigating a range of mental health and family support services throughout Bucks County, PA. The successful candidate will be an integral part of the Bucks County LIFE program and will assist with the Transition to Independence (TIP) program. Collaboration with county and healthcare services is key to delivering top-tier care to our clients.

The role encompasses supporting individuals and families as they navigate the complexities of various service systems, with responsibilities including case management, coordination, planning, referrals, and providing support. The focus will be on managing complex cases, utilizing principles of strength-based, person-centered care, and fostering recovery and resilience. Additionally, the Administrative Case Manager will organize and promote community events and educational groups, enhancing our outreach and community education efforts.

Schedule: Full time, Monday-Friday; typical office hours with some evenings and weekends as needed/on occasion for trainings, events, or emerging client needs.

Essential Functions of the Administrative Case Manager :

Client Assessment & Support:

  • Maintain a caseload of complex, multi-system cases providing coordination, referral, planning, and support.
  • Coordinate and provide care that is timely, effective, and person-centered.
  • Monitor clients in care, supporting youth, families, and caregivers in connecting to services/benefits and developing discharge and aftercare plans.
  • Assist clients in making informed care decisions and accessing resources and benefits.
  • Provide linkages of services for youth by identifying, developing, and utilizing informal support networks in individual service planning.
  • Work alongside youth and families in the community to provide education, empowerment, and resource information.
  • Partner with peer support for the case management outcomes of youth in RTF settings.
  • Plan, coordinate, and facilitate youth and family support groups and events for clients receiving services.

Communication & Collaboration:

  • Build and maintain collaborative relationships with county partners, community agencies, and care systems to ensure continuous excellent care.
  • Attend and facilitate interagency meetings for youth and families as needed by the Program Director and Child and Adolescent Social Service Program (CASSP) for open cases.
  • Attend and participate in weekly team meetings and case reviews with Family Support Partners and TIP Facilitators.

Administrative Tasks:

  • Respond to client and family inquiries via phone and/or email, conduct initial intake interviews in a timely and professional manner, and distribute incoming referrals to designated team members.
  • Organize and compile resources for populations receiving services.
  • Prepare and distribute program materials; participate in community education and outreach efforts to support agency and program education.
  • Assist with scheduling and planning marketing outreach and training to individuals, groups, families, and community members about the LIFE and TIP Programs.

Quality Assurance:

  • Assist leadership in maintaining quality assurance processes of client outcomes and maintain compliance under Blended Case Management guidelines.

Other:

  • Flex time between Bucks LIFE program and TIP Program as requested by leadership.
  • Maintain regular and predictable attendance, work scheduled overtime, or be available for unplanned overtime as necessary to meet program needs.
  • This position requires the ability to travel using a personal vehicle during a normal workday to successfully carry out the expectations of this position. This individual will be required to always drive safely, meet eligibility criteria of the Motor Vehicle Policy, and successfully pass the agency’s substance abuse screening at any time during employment.

Requirements

Requirements and Qualifications

Education:

  • Bachelor’s degree in psychology, social work, sociology, psychology, criminal justice, education, or related field. Is required.

Experience:

  • Two years of experience working directly with individuals who use mental health services (adults, children, or adolescents) is preferred but not required.

Knowledge, Skills, and Abilities:

  • Excellent oral and written communication skills.
  • Knowledge of systems serving children and adults, alongside a variety of disabilities and mental health conditions.
  • Ability to develop positive relationships with individuals receiving services, team members, and community partners.
  • Capacity to direct and prioritize own work and be flexible.
  • Proficiency in, or ability to learn and use, workplace applications, platforms and technologies.
