Whitecap Search is Hiring a RN Care Manager Near Niagara Falls, NY
WCS Healthcare is currently seeking a Care Management Nurse for an on-going contract assignment based out of Niagara Falls, NY. The Care Manager will work out of Niagara but traveling within neighboring counties is expected.
Job Function:
- Assess new patients
- Update and revise patient health care plans as needed
- Educate patients and families about health care plans and options
- Make clinical decisions for routine patient care
- Accurately document and submit medical records
- Demonstrate knowledge in medical care; such as diagnostic procedures, medication, symptoms, and other treatment-related therapies
- Complete a comprehensive assessment and develop a care plan for short-term & long-term needs utilizing clinical expertise
- Manages active cases based on case intensity and acuity
- Interacts continuously with member, family, physician(s), other providers and the interdisciplinary team
- Acts as liaison and member advocate between the member/family, physician and facilities/agencies
Job Requirements:
- Current New York State RN license (or willingness to obtain)
- BLS certification from the American Heart Association
- CCM certification preferred
- UAS exp required
- MLTC
- 2 years of MLTC care experience required
- Excellent communication and customer service skills
