JOB SUMMARY:
Profession: RN - Home Health Case Manager
Client: Direct Client
City: Monterey
Duration: 13 weeks (Extendable)
Shift: 8:00 AM to 5:00 PM. 8 hours. 40 hours a week.
Duties:
- Conduct patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed by the physician.
- Monitor patient conditions and update patient records.
- Educate patients and their families on proper healthcare strategies.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
- Provide emotional support and guidance to patients and their families.
REQUIREMENTS:
- MInimum 1 year of recent Med/Surg nurse experience or 1 - 2 years of recent Home Health Care experience.
- Active license in CA state.
- COVID Vaccinated.
- BLS / CPR Preferred.
