Job Description:
The Lead Care Manager (LCM) plays a pivotal role in our Enhanced Care Management (ECM) program in Mystis-Monterey County, responsible for overseeing the case management of members and their families. The primary goal of the LCM is to help members improve their health and overall well-being by ensuring they have access to the services and programs they need. The LCM will work closely with members, healthcare providers, and community resources to coordinate and facilitate the delivery of high-quality care.

Key Responsibilities:

• Collaborate with members and their families in Mystis-Monterey County to assess their healthcare needs, goals, and preferences.
• Develop and implement individualized care plans that address the unique needs of each member.
• Act as a liaison between members, healthcare providers, and community resources in Mystis-Monterey County to coordinate the delivery of services and programs.
• Utilize your Certified Nursing Assistant (CNA) or Licensed Vocational Nurse (LVN) skills to provide care coordination and support.
• Draw upon your Electronic Health Record (EHR) expertise to maintain accurate and up-to-date documentation of all interactions and care plans.
• Leverage your social work experience to provide emotional support and resources to clients with complex needs.
• Monitor and track members' progress towards their health and well-being goals.
• Provide education and support to members and their families regarding available services, benefits, and resources in Mystis-Monterey County.
• Advocate for members to ensure they receive appropriate care and services.
• Participate in regular team meetings and collaborate with other care managers to optimize care delivery.
• Stay up-to-date with industry trends and best practices in care management.

Qualifications:

• Certified Nursing Assistant (CNA) or Licensed Vocational Nurse (LVN) certification is required.
• Previous experience in care management, case management, or healthcare coordination is preferred.
• Strong communication and interpersonal skills.
• Ability to work both independently and as part of a team.
• Excellent problem-solving and decision-making abilities.
• Empathetic and compassionate attitude towards members and their families in Mystis-Monterey County.
• Knowledge of healthcare systems and community resources.
• Proficient in using electronic health records (EHR) and other relevant software.
• Experience in social work and dealing with clients with complex needs is a strong asset.
