Astrana Health is Hiring a Patient Care Specialist I Near Monterey, CA
- Carry out post-visit instructions from our internal healthcare providers
- Provide post-visit education and information to patients as needed
- Create authorization requests for referrals to specialists and other services
- Collect authorizations and medical records from various provider sources for proper documentation
- Send reports or notes to other providers and confirm receipt
- Upload records to the Provider Portal
- Assist with conducting various screening tests, including operating machines for bone density scan, retinal eye scan, and blood sugar testing
- Conduct outreach calls to inform patients of recommended services
- Promote disease prevention and chronic disease management messaging
- Schedule appointments for services with our internal team
- Assist patients with health questionnaires
- Input patient information accurately into electronic health record system
- Assist with administrative tasks for the department, including, but not limited to: receiving incoming calls regarding department programs, sending faxes to providers, receiving and redirecting incoming fax, uploading files
- If applicable, provide translation assistance in department programs
- Uses, protects, and discloses our company’s patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
- Associate’s Degree required
- 1 year experience in customer service, outreach calls, or health education
- 1 year experience working in healthcare/managed care environment, such as IPA, MSO, or health plan OR in health education
- General understanding of medical conditions and specialties
- Experience using Microsoft applications such as Word, Excel, and Outlook
- Equivalent combination of education and/or experience
- Must be able to project a professional, pleasant image while interacting with a variety of personalities and cultures
- Must demonstrate tact and diplomacy in interpersonal interactions
- Should have strong communication and customer service skills and respect for confidentiality
- Must have ability to plan, prioritize, and complete tasks
- Bachelor’s degree
- Bilingual in Chinese (Cantonese and/or Mandarin)
- Experience with Medicare STAR Metrics and HEDIS Measures
- Experience in clinical/medical settings
- Experience using EHR systems

- Our organization follows a hybrid work structure where the expectation is to work both in office and at home on a weekly basis, after a 90 day in-office training period. The office is located at 568 W. Garvey Ave., Monterey Park, CA 91754.
- The total compensation target pay range for this role is: $21.00 - $24.00 per hour. The salary range represents our national target range for this role.
