A. Therapy 
1. Conducts therapy groups for clients to meet established treatment goals
2. Provides effective therapy services to patients as delineated in the treatment plan
3. The intensity of service provided is consistent with the patient’s acuity. The treatment plan reflects changes in acuity
4. Provides individual therapy, family therapy and other services a outlined in the treatment plan
5. Develops and modifies an individualized treatment plan for each assigned patient and addresses the patient’s identified needs. Ensures that the treatment plan is carried out as intended. Modifies the treatment plan as feedback about the clients needs and strengths is made available
6. Conducts intakes/assessments

B. Documentation 
1. Ensures the client records accurately reflects the treatment taking place, the strategies delineated in the treatment plan are carried out and there is a logical flow to the treatment that has taken place based on the clinical information
2. Documentation is completed according to timelines and format delineated in the policy and procedures manuals for each type of client record entry. Documentation is written such The Centers personnel, third party reviewers and future providers can discern the plan, course and effectiveness of treatment
3. Logs all relevant phone calls in the progress notes delineating date and purpose of call

C. Coordination Of Services 
1. Provides linkage for intra and inter agency client processes
2. Informs others involved in the treatment of client of significant events or changes in treatment
3. Presents court testimony on behalf of client when required
4. Consults with school staff, parents and others to plan and integrate appropriate services

C. Productivity
1. Reaches Billable Hours of per month as defined by attached work plan.
2. Maintain and manage caseload as assigned.
