Monogram Kidney Clinic is seeking a full-time palliative care physician to treat members with CKD and ESRD. Treatment and oversight will occur primarily through telehealth and locally in person. The physician will connect with CKD and ESRD appropriate for conservative care and symptom management. They will also engage with patients who have multiple complex comorbidities. This is an opportunity to practice medicine in an evidence-based environment alongside nephrologists committed to raising renal care outcomes.
Monogram physicians are quality-driven and apply their clinical expertise in a managed care, population health context. Physicians engage in population health management in a sophisticated, outcomes-driven manner that promotes palliative care where clinically appropriate and focuses on patient education, autonomy, and quality of life.
This job is ideal for physicians who embrace the trends toward telehealth and value-based care; rewarding outcomes rather than utilization. The physician must have experience in geriatrics and end-of-life issues, advocating for conservative care where appropriate and where evidence-based criteria show that dialysis will not extend the duration of life.
The physician will work remotely from home unless needed to see patients in person. Monogram is a national practice. Physicians will be licensed in multiple states after joining the company. Physicians work on an employed basis, full-time, five days a week.
Roles & Responsibilities:
Build & Manage Patient Panel:
Collaborate with POD Medical Director to proactively identify CKD & ESRD members in predefined geographic regions to build a patient panel
Own the outcomes for the patient panel, driving both predetermined clinical and financial KPI's through interventions outlined below:
Drive Clinical Interventions for Patient Panel:
Treat panel of patients both in-home and virtually as necessary to drive healthcare outcomes
Apply evidence-based criteria in determining the appropriateness and necessity of care; in particular, in determining whether dialytic or non-dialytic care is the correct evidence-based pathway
Reduce pain in patients who have selected the palliative care pathway
Round on patient panel (if admitted) in selected hospitals within a defined geographic market
Reduce preventable episodes of care including hospital readmissions
Refer members to appropriate specialists to drive clinical care
Develop key relationships with local providers (PCP, Nephrologist, Specialists) to improve market alignment
Support Monogram Clinical Team:
Work with local Monogram POD's to drive KPI's
Participate in monthly sessions with the Regional Medical Director & CMO concerning KPI expectations
Backup coverage will be shared in the form of another Monogram MD
Physicians will maintain active privileges at local dialysis centers and hospitals
Review of Annual Wellness Assessments to ensure proper coding and provision of medical diagnosis
Assist in the training of NPs and other field providers
Requirements:
1 years treating patients with CKD and ESRD
Desire to practice in a renal care environment focused on population health management
Demonstrated experience applying evidence-based clinical criteria
Comfort and skill in end-of-life discussions
Experience with high-need Medicare Advantage and managed Medicaid populations
Expertise in managing medication panels for complex patients
Current state medical licensed without restrictions
Willingness to become licensed in multiple states
Doctorate degree in Medicine
Board-certified in IM/ Geriatric or Hospice & Palliative medicine
