Day to Day Life at Custom

• Provide care for our community of recovering patients, ushering them back to a life of independence.
• Conduct and complete initial assessment of ourpatients in order to generate a personalized plan of care in collaboration with physicians and the interdisciplinary team.
• Educate our patients and their families on next steps in continuing care and improvement of day to day life including: nutrition, healthy living, and treatments.
• Coordinate and perform medical treatments to enhance the well-being of the patient with the goal of enhancing patient health.
• Focus on the goal of improving the overall health of our community.
• Perform all duties with a passion for compassion.

Who you are

• Valid registered nursing license (required)
• 1 years of registered nursing experience 
• Associate's Degree in Nursing required
• A heart for providing the best care to our community
