Volunteers in Medicine Chattanooga is Hiring a Community Health Navigator Near Chattanooga, TN
Do you have a passion to help and make a real impact in the lives of others? Volunteers in Medicine, Chattanooga primary care clinic is seeking a Community Health Navigator to join our staff. This Flexible, Part Time position is responsible for helping patients and their families to navigate and access community services/resources. The CHN also coaches patients in effective management of chronic health conditions and self-care to adopt healthy behaviors. A working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services. An associate degree preferred with an emphasis or focus on social work / health prevention / advocacy and cultural awareness. Bilingual, preferably in Spanish a plus. Monday through Thursday dayshift, up to 24 hours weekly.
