Overview:
The Resource Nurse is responsible for assisting the Clinical Manager in the daily triage of the Team activities of accepting referrals, assuring timely admissions, assigning patients to the appropriate team members and monitoring patient care as ordered on the interdisciplinary plan of care.
Location: Prince William County, VA

Responsibilities:The Resource Nurse is responsible for assisting the Clinical Manager in the day-to-day triage of the Team activities of accepting referrals, assuring timely admissions, assigning patients to the appropriate team members and monitoring patient care as ordered on the interdisciplinary plan of care. The Resource Nurse participates in IDT meetings and assists in reviewing and completing the IDT meeting notes. The Resource Nurse assists with medical record audits to assure quality care and compliance with state and federal regulations, CHAP accreditation standards, and the organizations policies and procedures.
Experience Requirements
Must have three years of Hospice/Home Health experience. A minimum of two years of related management experience and at least four years of medical surgical and/or oncology is preferred.
Education Requirements
Must have an LPN education from an accredited nursing program. A BSN RN is preferred.
Required Certificates and/or Licenses
Must be licensed as a nurse with licensure from the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia.
Driver's License
Must have a driver's license in good standing and access to a vehicle during working hours.
