Description
Job Summary
This position provides system navigation and support for appropriate level of treatment for individuals with SUD, SMI, SED, and COD. Employee in this position assists program participants with resources for SDOH and with any other benefits to fulfil basic needs and enhance well-being.
What You Will Be Doing to Make a Difference...
  • Addresses barriers to SDOH and assist with obtaining SDOH benefits and any other community resources.
  • Engage individuals with SUD, SMI, SED, COD in the treatment system and assist with access to treatment for appropriate level of care.
  • Work collaboratively with other service providers, including PCP, and coordinate care meetings when needed.
  • Acting as a primary point of contact
  • Complete intakes, assessment, and treatment planning.
  • Performs outreach to newly referred individuals to encourage program participation.
  • Provides temporary assistance with transportation to individuals to any essential medical and behavioral services that align with wellness plan.
  • Develop and nurture collaborative relationships with external and internal providers.
  • Complete all required clinical documentation accurately and timely in agency's electronic medical record system.
  • Attend mandatory trainings, including EBP and all trainings necessary to acquire and maintain certification or licensure.
  • Appropriately and respectfully represent the agency at all times.
  • Participate in meetings as required, both internal and external.

Requirements
QUALIFICATIONS (Education and Experience)
  • Two years of experience working in the mental health field with individuals with SUD, SMI, SED and COD.
  • High school diploma as a minimum is required, BA in related field is preferred. Preferred certifications: Community Health Worker, Peer Specialist, Certified Recovery Coaching.
  • Sound knowledge and understanding of the healthcare system and different levels of care, esp. related to mental health and substance use disorders treatment.
  • Must possess strong organizational, communication and time management skills; able to work independently.
  • Experience with EMR system is desirable.
  • Local knowledge about Social Determinants of Health (SDOH) resources is required.
  • Knowledge, skills and supports to:
  • Engage difficult to reach, traumatized, disengaged or high-risk individuals.
  • Work with individuals with co-occurring, mental health and substance use conditions.
  • Identify needs of abused and neglected children and adults.
  • Collaborate with other disciplines and services.
  • All pathways to recovery.
  • Ability to multitask and be adaptable.
  • Ability to develop and maintain rapport with personally and culturally diverse clients.
  • Have strong interpersonal skills and ability to build trust with community partners.
  • Practice trauma informed care, person-centered and recovery-oriented treatment modalities.
  • Must work well independently and is a self-starter with strong organizational skills.
  • Must exhibit proactive communication when working in team dynamics with multiple disciplines and collaterals.
  • Possess computer proficiency and experience with telehealth.
  • Must have a valid MA driver's license with acceptable driving record and current proof of insurance.
  • Acceptable CORI and SORI background record check as required by program.
