On-Call LPN – 7 On / 7 Off Jefferson City / Columbia, Missouri Region

Three Rivers Hospice is looking for a compassionate, dependable On-Call LPN to join our growing hospice team serving the Jefferson City and Columbia region.
This position offers a 7 days on / 7 days off schedule, giving you the opportunity to provide meaningful care to patients and families while enjoying a consistent week away from work.
Position Responsibilities
- Provide after-hours hospice nursing support and symptom management
- Respond to patient and family needs with compassion and professionalism
- Perform PRN visits as needed throughout the service area
- Provide telephone triage and clinical support
- Collaborate with physicians and the interdisciplinary hospice team
- Communicate effectively with patients, families, facilities, and caregivers
- Complete accurate and timely clinical documentation
- Respond calmly and confidently to urgent patient-care situations
Qualifications
- Current Missouri LPN license
- Hospice experience preferred, but not required
- Strong assessment, critical-thinking, and communication skills
- Compassionate and professional bedside manner
- Ability to work independently and make sound clinical decisions
- Comfortable responding to after-hours and urgent patient needs
- Valid driver’s license and reliable transportation
