Three Rivers Hospice is seeking a compassionate and dependable On-Call Licensed Practical Nurse (LPN) to join our Jefferson City team and serve patients throughout Jefferson City and the surrounding service areas.
This role is perfect for an LPN who is confident working independently and is passionate about providing after-hours care, comfort, and support to patients and families during critical moments.
As the On-Call LPN, you will serve as a vital connection between patients, families, and the hospice team—ensuring needs are addressed promptly with compassion and professionalism.
Schedule
- 7 Days On / 7 Days Off

What You’ll Do
- Serve as the first point of contact for patient and family needs after hours
- Respond to calls and provide in-home and facility-based visits as needed
- Deliver hands-on care and assist with symptom management and comfort measures
- Communicate changes in patient condition to the RN and interdisciplinary team
- Provide emotional support and guidance to patients and families
- Complete documentation accurately and in a timely manner
- Travel throughout Jefferson City and surrounding service territories to ensure prompt response
What We’re Looking For
- Active Missouri LPN license
- Previous nursing experience required (hospice experience preferred)
- Strong clinical judgment and ability to remain calm under pressure
- Compassionate communication skills
- Reliable transportation, valid driver’s license, and current auto insurance
