What You'll Do
- Manage an individualized caseload of hospice patients in their homes, assisted living communities, and nursing facilities.
- Perform comprehensive nursing assessments and coordinate patient care.
- Develop and implement personalized plans of care in collaboration with physicians and the interdisciplinary team.
- Provide expert symptom management and patient advocacy.
- Educate patients and families on disease progression, medications, and end-of-life care.
- Complete accurate and timely clinical documentation.
- Participate in interdisciplinary team meetings and quality improvement initiatives.
Qualifications
- Current Missouri Registered Nurse (RN) license.
- Previous nursing experience required; hospice, home health, or case management experience preferred.
- Strong assessment, communication, and organizational skills.
- Compassionate, patient-centered approach to care.
- Valid driver's license, reliable transportation, and current auto insurance.
