Three Rivers Hospice is seeking a compassionate and dedicated Hospice Social Worker to join our interdisciplinary team serving St. Joseph and the surrounding North Missouri communities.
As a Hospice Social Worker, you will play a vital role in helping patients and families navigate the emotional, social, and practical challenges of end-of-life care. You'll advocate for each patient's wishes, provide compassionate counseling, connect families with valuable community resources, and help ensure every patient receives care with dignity and respect.

Responsibilities
- Complete comprehensive psychosocial assessments and develop individualized plans of care.
- Provide emotional support, counseling, and grief support to patients and their families.
- Advocate for each patient's goals, values, and end-of-life wishes.
- Assist patients and caregivers in identifying community resources, financial assistance, and support services.
- Collaborate with physicians, nurses, chaplains, aides, volunteers, and the interdisciplinary hospice team to provide holistic patient-centered care.
- Support patients and families through crisis situations using effective intervention and counseling techniques.
- Maintain accurate, timely documentation including assessments, care plans, and progress notes.
- Educate patients and caregivers regarding hospice services and available resources.
- Ensure compliance with HIPAA regulations and all federal and Missouri hospice regulations.
Qualifications
- Master's Degree in Social Work (MSW) from a Council on Social Work Education (CSWE) accredited program; or
- Bachelor's Degree in Social Work (BSW) with at least one year of social work experience in a healthcare setting and supervision by an MSW as required by regulation.
- Current knowledge of Missouri and Federal hospice regulations.
- Commitment to the hospice philosophy of comfort-focused, compassionate end-of-life care.
- Strong communication, organizational, and interpersonal skills.
- Ability to work independently while collaborating effectively within an interdisciplinary team.
- Experience with crisis intervention, grief counseling, motivational interviewing, and working with diverse populations is preferred.
- Working knowledge of electronic documentation systems and basic computer skills.
