Three Rivers Hospice is seeking a compassionate and dependable Licensed Practical Nurse (LPN) Case Manager to join our team in Cassville.

What You’ll Do
- Provide in-home and facility-based nursing visits to hospice patients
- Monitor patient conditions and communicate changes to the RN Case Manager and care team
- Assist in carrying out the plan of care focused on comfort and quality of life
- Administer medications and treatments as ordered
- Provide education, guidance, and emotional support to patients and families
- Complete documentation accurately and in a timely manner
- Collaborate with the interdisciplinary hospice team to ensure excellent patient care
What We’re Looking For
- Active Missouri LPN license
- Previous nursing experience required (hospice or home health preferred)
- Strong communication skills and compassionate bedside manner
- Ability to work independently and manage your daily schedule
- Reliable transportation, valid driver’s license, and current auto insurance
