What You'll Do
- Manage a caseload of hospice patients in their homes, assisted living communities, and nursing facilities.
- Provide skilled nursing visits focused on comfort, dignity, and quality of life.
- Monitor patient conditions and communicate changes with the RN Case Manager, physician, and interdisciplinary team.
- Administer medications and treatments as ordered.
- Educate patients and families regarding disease progression, symptom management, and hospice services.
- Maintain accurate and timely clinical documentation.
- Collaborate with nurses, CNAs, social workers, chaplains, and other hospice team members to provide exceptional patient care.
Qualifications
- Current Missouri Licensed Practical Nurse (LPN) license.
- Previous nursing experience required; hospice, home health, long-term care, or case management experience is preferred.
- Strong communication, assessment, and organizational skills.
- Compassionate, dependable, and patient-focused.
- Valid driver's license, reliable transportation, and current auto insurance.
