Medicaid-Homecare Services

Position: Certified Nurse Assistant in the home care setting, primarily working in clients' residences.

Responsibilities

• Provide direct patient care, including personal hygiene, transfers, repositioning, and ambulation assistance.
• Administer basic nursing care and prescribed exercises under supervision.
• Prepare meals considering dietary needs and preferences.
• Offer companionship and engage clients in activities tailored to their interests.
• Perform light housekeeping duties and maintain accurate documentation of care activities.
• Monitor and report changes in clients' physical or psychosocial conditions.
• Follow the care plan strictly, including documentation via EVV app or sheet.
• Ensure confidentiality, uphold patient rights, and report any abuse or neglect.
• Assist clients in promoting independence within their capabilities.
  
  

Qualifications & Skills

• Current certification with DFS.
• High school diploma or GED preferred.
• Effective communication skills and ability to follow directions.
• Physical ability to perform tasks such as lifting approximately 50 lbs, bending, and standing.
• Compassionate attitude towards patient care.
  
  

Additional Info: The role involves travel within a designated area, with variable caseload and no guaranteed hours. Equipment used includes blood pressure cuffs and stethoscopes. The CNA is responsible for ensuring high-quality care and adherence to policies.
