What are the responsibilities and job description for the Case Manager position at LHH?
We are seeking a compassionate and proactive Case Manager with a strong understanding of harm reduction principles to support individuals experiencing homelessness. This role requires direct engagement with participants in a dynamic environment where flexibility, empathy, and cultural competence are essential.
Responsibilities
- Maintain a visible presence by interacting directly with residents, fostering trust and rapport.
- Conduct regular check-ins and provide individualized support to participants, including those with mental health challenges, substance use disorders, or domestic violence histories.
- Utilize harm reduction strategies to meet participants where they are, reduce risks, and promote safety without judgment.
- Take initiative to identify and address participant needs without waiting for direction.
- Assist with participant intakes as needed, adapting to shifting priorities and emergencies.
- Accurately record all interactions, incidents, and efforts to support participants with strong emphasis on thorough case notes.
- Help participants move from interim to permanent housing by guiding them through documentation and connecting them to resources.
Qualifications
- Minimum 1 year of case management experience with the homeless population.
- Bilingual (Spanish) preferred to effectively serve diverse communities.
- Knowledge of harm reduction principles and trauma-informed care.
- Must have a valid driver license (no more than 2 points) and reliable transportation
- Ability to thrive in a fast-paced environment with shifting priorities.
Schedule: Sunday- Thursday 9am-6pm Onsite
Pay Range: $46Kโ$49.9K per hour
Location: San Pedro, CA 90731 Onsite
