What are the responsibilities and job description for the Case Manager position at LHH?
We are seeking a Case Manager who is adaptable, solution-focused, and comfortable working in a fast-paced, high-needs environment.
What You’ll Do:
- Manage a caseload of approximately 20–25 participants, providing short term, goal-oriented case management
- Complete intake assessments and develop individualized service plans focused on housing navigation, healthcare access, mental health support, income stability, and life skills
- Coordinate and refer participants to community resources (DPSS, SSI, healthcare providers, food assistance, housing programs, etc.)
- Support participants in developing routines, problem solving skills, and readiness for permanent housing
- Collaborate closely with on-site staff to ensure program compliance, safety, and continuity of care
What We’re Looking For:
- Strong ability to function effectively in an on-site, high traffic environment with frequent interruptions
- At least 2 years of experience working with vulnerable populations, preferably in interim housing, homelessness services, shelters, or social services
Location: Long Beach, CA 90806 (Onsite)
Schedule: Monday to Friday 8AM-4:30PM
Pay: $24-26/hr. DOE
3 month contract covering for a leave
