Key Responsibilities:
- Provide direct clinical care including symptom and pain management, vital signs, medication monitoring, and wound care
- Collaborate with the interdisciplinary care team—physicians, social workers, therapists—to develop and implement personalized care plans
- Offer emotional support and education to patients and families navigating end-of-life care
- Maintain accurate and timely documentation in compliance with hospice standards
- Be available 24/7 for on-call needs, including nights, weekends, and holidays, to ensure seamless patient support
Qualifications:
- Active LPN license in Minnesota
- Prior experience in hospice, palliative, nursing home, or home health settings is required.
- Strong clinical judgment paired with empathy, patience, and excellent communication skills
- Comfortable working autonomously in patients’ homes and coordinating care across disciplines
